Post-mortem blameless review template
Overview
Blameless post-mortems focus on fixing systems and processes first. They are not consequence-free for repeated negligence, but they should not devolve into public shaming of individuals.
Template sections
- Incident summary with a visual timeline.
- Quantified customer or business impact.
- Detection path: alerts, analyst actions, third-party notices.
- Response highlights and misses during contain and eradicate phases.
- Root causes (usually multiple: technical and procedural).
- What went well—reinforce behaviors to keep.
- Action items with a single owner, due date, and measurable verification.
Facilitation
Keep rank from silencing junior contributors; timebox complaint spirals; use a neutral facilitator when emotions run high.
Follow-through
Track action items in a visible system (for example Jira); executive review aging items weekly until closed.
Communication
Optional sanitized external blog or client newsletter only after legal review—maturity messaging, not finger-pointing.
Anti-pattern
A polished PDF post-mortem that nobody reads. Pin a short executive summary where teams actually work.
Metrics
Compare mean time to detect and respond before the incident versus six months after remediation work completes.
People
Publicly thank on-call responders. Burnout is a second, quieter incident—treat it as a risk.
